Service Request

 

The ID-ASD-PD Access Desk receives and processes applications for CLSC services and rehabilitation services for clients with intellectual disabilities (ID), autism spectrum disorders (ASD) or physical disabilities (PD).

How to request DP services

Intellectual disability and autism spectrum disorders

Services in CLSC and rehabilitation centres

To access services go to your local CLSC’s psychosocial department or main reception desk.

A professional will guide you through the process, and follow-up with the Access Desk at your CIUSSS. The Access Desk will give you an answer in writing.

Physical disabilities

To access services go to your local CLSC’s psychosocial department or main reception desk.

A professional will guide you through the process, and follow-up with the Access Desk at your CIUSSS. The Access Desk will give you an answer in writing.

Rehabilitation service

If a health professional has confirmed that you require rehabilitation services for your condition:

1. Fill out the following documents:

Request for Services form (WORD)
Consent to the communication of personal information in the context of the integrated service network for persons with disabilities (PDF)

Notes: Other documents may be needed to process a request for services.

2. Forward the completed documents to the Access Desk or to the facility offering the
